Introduction

Fungal infections are diseases caused by fungi that invade or grow on the human body. Fungi are microscopic organisms that exist naturally in the environment, including in soil, water, plants, animals, and on human skin. Most fungi are harmless, and many normally live on the skin or mucous membranes without causing disease. However, under favorable conditions, certain fungi can multiply excessively or invade deeper tissues and produce infection.

Fungal infections range from mild superficial conditions affecting the skin and nails to serious infections involving the lungs, bloodstream, brain, or multiple organs. Their clinical presentation depends greatly on the type of fungus, the site of infection, the patient's immune status, and the route through which the organism enters the body.

Superficial fungal infections are extremely common. They include infections such as athlete's foot, ringworm, jock itch, and fungal infections of the nails. Mucosal infections such as oral thrush and vulvovaginal candidiasis are also frequent. These infections may cause itching, redness, scaling, discharge, discomfort, cracking, or changes in the appearance of affected tissues.

More serious fungal infections are less common but can be potentially life-threatening. Opportunistic fungi such as Candida, Aspergillus, and Cryptococcus can cause invasive disease particularly in people with weakened immune systems. Patients receiving chemotherapy, organ or stem-cell transplant recipients, individuals with advanced HIV infection, and critically ill hospitalized patients may be at increased risk.

Understanding fungal infections requires knowledge of fungal biology, transmission, risk factors, clinical manifestations, diagnostic methods, antifungal therapy, and preventive measures. Although many superficial infections can be successfully treated, inappropriate treatment can result in persistent disease, recurrence, complications, or antifungal resistance.

Understanding Fungi

Fungi belong to a separate biological kingdom and differ from bacteria, viruses, and parasites in important ways. They are eukaryotic organisms, meaning their cells contain a nucleus and membrane-bound organelles. Their cell walls contain substances such as chitin and glucans, while their cell membranes contain ergosterol.

These structural characteristics are clinically important because they provide targets for antifungal medications. For example, several antifungal drugs interfere with ergosterol synthesis or damage the fungal cell membrane. Other agents interfere with fungal cell-wall synthesis.

Fungi can exist in several forms. Yeasts are generally single-celled organisms that reproduce by budding. Candida is an important example. Molds are multicellular organisms composed of branching structures called hyphae. Dermatophytes are filamentous fungi that grow in keratinized tissues such as the skin, hair, and nails.

Some fungi are dimorphic, meaning they can exist in different forms depending on environmental conditions. Certain medically important dimorphic fungi may exist as molds in the environment but transform into yeast-like forms within human tissues. This ability can contribute to their capacity to cause systemic disease.

Not every exposure to a fungus results in infection. The human body has multiple protective mechanisms, including intact skin, mucosal barriers, normal bacterial flora, innate immune responses, and adaptive immunity. Disease develops when fungal growth exceeds the body's ability to control it or when fungi penetrate normally protected tissues.

Major Types Of Fungal Infections

Fungal infections can be classified according to the anatomical depth and tissue involved. This classification is clinically useful because superficial infections generally behave differently from invasive fungal diseases.

Superficial and cutaneous fungal infections primarily involve the outer layers of the skin, hair, and nails. Dermatophyte infections are among the most common examples. They typically remain within keratinized tissues because dermatophytes use keratin as a nutrient source.

Subcutaneous fungal infections extend deeper into the skin and subcutaneous tissues. They are frequently associated with traumatic implantation of fungi from soil, vegetation, or contaminated material.

Systemic fungal infections can involve internal organs after fungi enter the body, commonly through inhalation or, in some situations, through direct inoculation. Some systemic fungi can cause disease even in otherwise healthy individuals, whereas others predominantly affect people with impaired immunity.

Opportunistic fungal infections occur when normally controlled organisms or environmental fungi cause disease in individuals whose immune defenses are compromised. These infections can become severe and may involve multiple organs.

Common Superficial Fungal Infections

Superficial fungal infections are among the most frequently encountered fungal diseases in clinical practice. They commonly affect warm, moist areas of the body and may spread through direct contact or contaminated objects.

Dermatophytes are responsible for several common infections collectively called tinea. The term "tinea" is followed by the body site involved. Tinea pedis affects the feet, tinea corporis affects the body, tinea cruris affects the groin, and tinea capitis affects the scalp.

Despite the common term "ringworm," these infections are not caused by worms. The name comes from the characteristic ring-shaped appearance that some lesions develop.

Tinea Pedis And Athlete's Foot

Tinea pedis, commonly known as athlete's foot, is a fungal infection of the feet. It is particularly common in environments where people frequently walk barefoot in communal areas such as changing rooms, swimming-pool surroundings, showers, and gyms.

The infection often develops between the toes. The skin may become white, macerated, cracked, scaly, itchy, and painful. In other cases, a dry scaling pattern may extend across the soles and sides of the feet.

Some patients develop vesicles or small fluid-filled blisters, particularly during inflammatory episodes. Severe cases may cause fissures that provide an opportunity for secondary bacterial infection.

Excessive sweating, prolonged use of occlusive footwear, and humid environments can promote fungal growth. The infection can also spread from the feet to other parts of the body, including the groin.

Treatment generally involves appropriate topical antifungal therapy for localized disease. More extensive or resistant infections may require systemic treatment depending on the causative organism and clinical circumstances.

Keeping the feet dry, changing socks regularly, allowing footwear to dry completely, and avoiding shared towels or footwear can reduce recurrence.

Tinea Corporis And Ringworm

Tinea corporis is a dermatophyte infection involving the trunk, limbs, or other areas of the body excluding the scalp, groin, and feet.

Lesions often begin as small scaly areas and gradually expand outward. The edge may become more active, red, raised, and scaly while the center becomes relatively clearer. This can produce the characteristic ring-like appearance.

Itching is common, although its severity varies. Multiple lesions may occur, and lesions can enlarge if treatment is delayed.

Transmission may occur through direct contact with an infected person or animal, or through contaminated objects. Some dermatophytes can be acquired from pets, especially cats and dogs.

Diagnosis can often be suspected clinically, but laboratory testing may be helpful when the appearance is atypical or when treatment fails.

Tinea Cruris And Jock Itch

Tinea cruris is a dermatophyte infection affecting the groin and adjacent skin. It is commonly known as jock itch.

The infection usually produces an itchy, red, scaly rash in the groin folds. The margins may be more prominent than the center, and the rash can extend onto the upper thighs.

Warmth, sweating, tight clothing, obesity, and prolonged moisture can increase the likelihood of infection. Tinea cruris may occur together with tinea pedis because fungi can spread from infected feet to the groin through clothing or scratching.

A useful preventive strategy is to treat associated fungal infection of the feet and maintain good skin hygiene while keeping the groin area dry.

Tinea Capitis

Tinea capitis is a fungal infection of the scalp and hair shafts. It is particularly common in children.

Clinical findings can vary considerably. Some patients develop areas of scaling and hair loss, while others develop inflammation, broken hairs, or swollen lymph nodes.

A severe inflammatory reaction can produce a kerion, which is a painful, boggy, swollen lesion that may resemble a bacterial abscess. Kerion can potentially result in permanent scarring and hair loss if severe inflammation damages hair follicles.

Unlike many localized skin infections, tinea capitis generally requires systemic antifungal therapy because topical medications alone do not adequately penetrate infected hair shafts.

Close contacts and household members may sometimes require evaluation, particularly when infection is persistent or recurrent.

Fungal Nail Infections

Fungal infection of the nails, known as onychomycosis, is common and can be difficult to eradicate.

Affected nails may become thickened, brittle, discolored, distorted, or separated from the nail bed. The nail may develop white, yellow, brown, or other abnormal coloration.

Toenails are affected more commonly than fingernails. Fungal nail infection can coexist with athlete's foot, allowing the fungus to persist between the skin and nail.

Treatment can be prolonged because nails grow slowly. Depending on the organism, extent of involvement, and patient factors, therapy may involve topical antifungals or oral antifungal medications.

Laboratory confirmation is often valuable before prolonged systemic therapy because several non-fungal nail disorders can produce similar appearances.

Candidiasis

Candida species are yeast-like fungi that can normally live on human skin and mucosal surfaces. Under certain conditions, however, Candida can multiply excessively and cause infection.

Candida albicans is the most frequently encountered species, although other Candida species can also cause disease.

Candidiasis can affect the mouth, skin folds, genital tract, and bloodstream. The clinical significance ranges from mild localized disease to severe invasive infection.

Factors that can encourage Candida overgrowth include antibiotic exposure, diabetes, immune suppression, disruption of normal microbial flora, moisture, and certain medical devices.

Oral Thrush

Oral candidiasis, commonly called oral thrush, involves excessive growth of Candida in the mouth.

Patients may develop creamy white or yellowish plaques on the tongue, inner cheeks, palate, gums, or other oral surfaces. Burning, soreness, altered taste, or difficulty eating may occur.

When the plaques are gently removed, the underlying tissue may appear red and irritated. In some patients, especially those with significant immune compromise, infection can extend into the esophagus.

Infants, older adults, people using inhaled corticosteroids, individuals receiving antibiotics, and people with impaired immunity may have increased risk.

Treatment depends on severity and underlying risk factors. Recurrent oral candidiasis should prompt consideration of factors that may be contributing to repeated episodes.

Vulvovaginal Candidiasis

Vulvovaginal candidiasis is a common fungal infection involving the vulva and vagina.

Typical symptoms include intense itching, vulvar irritation, redness, burning, and abnormal vaginal discharge. The discharge is often described as thick and white, although symptoms and appearance can vary.

Some individuals experience burning during urination because irritated external tissues are exposed to urine. Pain during sexual intercourse may also occur.

Risk can increase during pregnancy, after antibiotic exposure, and in people with poorly controlled diabetes. Recurrent episodes require careful assessment because repeated symptoms are not always caused by Candida.

Correct diagnosis is important because bacterial vaginosis, sexually transmitted infections, dermatological conditions, and other disorders can produce similar symptoms.

Cutaneous Candidiasis

Candida can infect moist skin folds, producing a condition known as cutaneous candidiasis or candidal intertrigo.

Common locations include the groin, armpits, beneath the breasts, abdominal folds, and other areas where skin surfaces remain in prolonged contact.

The affected skin may be red, moist, macerated, and painful. Small satellite papules or pustules may appear around the main affected area.

Moisture, friction, obesity, diabetes, and prolonged skin occlusion can contribute to disease.

Management involves antifungal treatment along with measures to reduce moisture and friction. Simply treating the fungus without addressing persistent moisture may result in recurrence.

Invasive Candidiasis

Invasive candidiasis is a serious form of Candida infection in which the organism enters normally sterile sites, particularly the bloodstream.

Candidemia refers specifically to Candida infection of the bloodstream. It is an important healthcare-associated infection and can lead to dissemination to organs such as the kidneys, liver, spleen, eyes, heart, and brain.

Patients at increased risk include those in intensive care, individuals with central venous catheters, people receiving broad-spectrum antibiotics, patients undergoing major abdominal surgery, and individuals with significant immune suppression.

Clinical manifestations can be nonspecific. Fever and chills that persist despite antibacterial therapy may occur, but no single symptom reliably identifies invasive candidiasis.

Because invasive candidiasis can progress rapidly, early recognition and appropriate antifungal treatment are important.

Aspergillosis

Aspergillosis is caused by fungi belonging to the genus Aspergillus. These molds are widespread in the environment, and their spores can be inhaled.

Healthy individuals generally eliminate inhaled spores without developing serious disease. However, certain individuals can develop allergic disease, chronic pulmonary disease, or invasive aspergillosis.

Allergic bronchopulmonary aspergillosis is characterized by an exaggerated immune response to Aspergillus in the airways and occurs particularly in susceptible individuals with asthma or certain chronic airway conditions.

Chronic pulmonary aspergillosis may occur in individuals with pre-existing lung abnormalities. Symptoms can include chronic cough, fatigue, weight loss, and sometimes coughing up blood.

Invasive aspergillosis is particularly dangerous in severely immunocompromised patients. The fungus can invade blood vessels and spread from the lungs to other organs.

Cryptococcosis

Cryptococcosis is caused mainly by Cryptococcus species. Infection commonly begins after inhalation of fungal particles.

In immunocompromised individuals, particularly those with severe impairment of cellular immunity, the organism can disseminate from the lungs to the central nervous system.

Cryptococcal meningitis is an important manifestation. Symptoms may develop gradually and can include headache, fever, nausea, altered mental status, and neurological abnormalities.

Because the presentation may be less dramatic than bacterial meningitis, a high degree of clinical suspicion is important in high-risk patients.

Diagnosis can involve detection of cryptococcal antigen, microscopic examination, culture, and examination of cerebrospinal fluid when central nervous system involvement is suspected.

Histoplasmosis

Histoplasmosis is caused by Histoplasma species and is associated with exposure to fungal spores in contaminated environmental material.

The lungs are commonly involved because infection frequently follows inhalation.

Many infections are asymptomatic or mild, but severe disease can occur, particularly in people with impaired immunity. Disseminated infection can affect multiple organs.

Symptoms may include fever, cough, fatigue, weight loss, and respiratory difficulty. Disseminated disease may produce systemic manifestations involving the reticuloendothelial system and other organs.

Diagnosis may require a combination of antigen testing, antibody testing, culture, microscopy, and tissue examination depending on the clinical presentation.

Coccidioidomycosis

Coccidioidomycosis is caused by Coccidioides species and is acquired primarily through inhalation of airborne fungal particles.

Many infected individuals remain asymptomatic or develop a respiratory illness resembling influenza or community-acquired pneumonia.

Symptoms may include cough, fever, fatigue, chest discomfort, shortness of breath, and muscle or joint aches.

In some patients, infection can become chronic or disseminate outside the lungs. Disseminated disease can involve the skin, bones, joints, or central nervous system.

The geographic distribution of Coccidioides is an important consideration when assessing patients with compatible symptoms and relevant environmental exposure.

Sporotrichosis

Sporotrichosis is caused by Sporothrix species and traditionally has been associated with traumatic inoculation from plants, soil, or organic material.

The classic form begins at the site of inoculation as a small painless or mildly painful lesion. Additional lesions may develop along lymphatic channels, producing a characteristic ascending pattern.

The infection can occasionally spread beyond the skin, particularly in individuals with certain underlying risk factors.

Diagnosis is generally based on fungal culture and clinical findings. Treatment depends on the extent and type of infection.

How Fungal Infections Spread

Fungal transmission varies according to the organism.

Some fungi spread through direct skin-to-skin contact. Others are transmitted through contaminated clothing, towels, bedding, footwear, combs, or personal equipment.

Certain dermatophytes can spread from infected animals to humans. Pets may carry fungi without obvious disease, making veterinary assessment important when recurrent household infections occur.

Environmental fungi are often acquired through inhalation of spores. In these situations, person-to-person transmission is usually not the primary mechanism.

Subcutaneous fungal infections may occur when contaminated material penetrates the skin through a wound or puncture.

Candida infections are somewhat different because Candida can already be part of the body's normal microbial flora. Disease often results from overgrowth or invasion when local or systemic defenses are disrupted rather than from acquisition of the organism from another person.

Risk Factors For Fungal Infections

Several factors increase susceptibility to fungal disease.

A weakened immune system is one of the most important risk factors for invasive fungal infection. Conditions that impair immune function can reduce the body's ability to contain organisms that would otherwise remain harmless.

Diabetes can increase susceptibility to several fungal infections, particularly when blood glucose is poorly controlled.

Recent or prolonged antibiotic use can disrupt normal bacterial flora and allow certain fungi to proliferate.

Corticosteroids and other immunosuppressive medications can increase susceptibility to fungal disease. The degree of risk depends on the medication, dose, duration, and patient's overall immune status.

Prolonged hospitalization, intensive care treatment, central venous catheters, major surgery, and certain invasive procedures can increase the risk of invasive candidiasis.

Warm and humid environments promote many superficial fungal infections. Excessive sweating and occlusive footwear can contribute to athlete's foot, while persistent moisture within skin folds can promote candidal infection.

Skin trauma can create an entry point for fungi, particularly in subcutaneous infections.

Signs And Symptoms

The symptoms of fungal infection depend heavily on the affected body site.

Skin infections commonly produce itching, scaling, redness, cracking, burning, or changes in pigmentation. Some lesions develop raised borders or circular patterns.

Nail infections can produce thickening, discoloration, brittleness, distortion, and separation of the nail from its underlying tissue.

Oral infections may cause white plaques, soreness, burning, and difficulty eating.

Genital candidiasis can cause itching, irritation, redness, burning, and abnormal discharge.

Pulmonary fungal infections may produce cough, fever, chest discomfort, shortness of breath, fatigue, and weight loss.

Invasive fungal infections can cause nonspecific systemic manifestations such as persistent fever, chills, weakness, low blood pressure, or organ-specific abnormalities.

Neurological fungal infections may present with headache, confusion, altered consciousness, seizures, or other neurological signs.

Because many symptoms overlap with bacterial, viral, inflammatory, and noninfectious diseases, laboratory confirmation is often necessary for deeper or unusual infections.

Diagnosis Of Fungal Infections

Diagnosis begins with a detailed clinical history and physical examination.

Important historical information includes the duration of symptoms, previous episodes, recent antibiotic use, medication history, immune status, travel or residence in endemic regions, occupation, exposure to animals, environmental exposure, and contact with infected individuals.

For superficial infections, direct microscopic examination of skin scrapings, nail material, or other specimens can provide rapid evidence of fungal elements.

Potassium hydroxide preparation, commonly called a KOH examination, can dissolve keratin and make fungal structures easier to visualize microscopically.

Fungal culture can identify the organism and may be particularly useful when the diagnosis is uncertain or treatment has failed.

Molecular diagnostic methods may also be used in selected settings. Antigen and antibody tests can be useful for particular systemic fungal infections.

For invasive disease, blood cultures, tissue biopsy, histopathological examination, antigen detection, molecular testing, and imaging may all contribute to diagnosis.

Microscopic Examination And KOH Testing

KOH microscopy is a practical diagnostic method for many superficial fungal infections.

A specimen is collected from the active edge of a skin lesion, affected nail material, or other appropriate site. Potassium hydroxide helps clear keratin and cellular debris, allowing fungal structures to become more visible under the microscope.

Dermatophytes may appear as branching, septate hyphae. Yeasts may appear as budding cells, and Candida can sometimes demonstrate pseudohyphae.

The sensitivity of the test depends on specimen quality, timing, examiner experience, and prior treatment. A negative result does not always exclude fungal infection when clinical suspicion remains high.

Fungal Culture

Fungal culture allows organisms to grow under laboratory conditions and can help identify the causative species.

Culture is particularly useful for infections involving nails, scalp, unusual lesions, recurrent disease, and suspected invasive infections.

One limitation is that fungal cultures can take considerably longer than routine bacterial cultures. Some fungi grow relatively slowly, meaning results may not be immediately available.

Culture results should therefore be interpreted together with clinical findings and other laboratory investigations.

Biopsy And Histopathology

When fungal infection involves deeper tissues or has an unusual appearance, tissue biopsy may be necessary.

Histopathological examination can demonstrate fungal structures within tissue and reveal the host's inflammatory response.

Special stains can improve visualization of fungal elements. Depending on the suspected organism and specimen, laboratories may use stains such as periodic acid-Schiff or Gomori methenamine silver.

Biopsy can also help distinguish fungal infection from malignancy, inflammatory disorders, bacterial infection, and other conditions that mimic fungal disease.

Treatment Principles

Treatment depends on the type of fungus, anatomical location, severity of disease, immune status of the patient, and susceptibility of the organism.

Superficial infections can often be managed with topical antifungal agents. These medications are applied directly to the affected area and are useful when infection is limited to the skin.

Systemic antifungal treatment may be necessary when infection involves the scalp, nails, deep tissues, bloodstream, lungs, brain, or multiple organs.

Common antifungal classes include azoles, polyenes, echinocandins, allylamines, and other specialized agents.

The choice of medication should be based on the suspected or confirmed organism and the clinical situation rather than simply selecting the strongest available antifungal.

Topical Antifungal Therapy

Topical antifungals are commonly used for localized dermatophyte infections.

Agents such as terbinafine, clotrimazole, miconazole, and other topical preparations can be effective against appropriate superficial fungal infections.

Treatment duration varies according to the drug, site, organism, and severity of disease. Stopping therapy immediately after symptoms improve can contribute to persistence or recurrence.

The medication should generally be applied according to the prescribed instructions, including the recommended treatment duration.

Topical treatment is less effective for infections involving deep tissues, hair shafts, or extensive nail disease because adequate drug concentrations may not reach the infected structures.

Oral Antifungal Therapy

Systemic antifungal medications are used when topical treatment is inadequate or when the infection involves sites that require systemic drug penetration.

Terbinafine is widely used for certain dermatophyte infections, particularly some nail infections.

Azole medications such as itraconazole and fluconazole are used for various fungal infections depending on the organism and clinical setting.

For severe invasive infections, medications such as amphotericin B, voriconazole, isavuconazole, and echinocandins may be used depending on the specific diagnosis.

Systemic antifungal drugs can interact with other medications and may produce adverse effects. Some require monitoring of liver function or other laboratory parameters.

Antifungal Resistance

Antifungal resistance is an increasingly important clinical problem.

Resistance can develop when fungi acquire mechanisms that allow them to survive exposure to antifungal medications. Repeated or inappropriate exposure to antifungal agents can contribute to selection of resistant organisms.

Candida species with reduced susceptibility to azole drugs are an important example. Some Candida species can also demonstrate resistance to other antifungal classes.

Resistance may result in persistent infection, treatment failure, prolonged hospitalization, and increased healthcare costs.

For this reason, antifungal therapy should be selected carefully, and susceptibility testing may be necessary for certain invasive or treatment-refractory infections.

Complications

Many superficial fungal infections are uncomfortable but not life-threatening. However, complications can occur when infections are severe, neglected, recurrent, or improperly treated.

Persistent scratching can damage the skin and create openings through which bacteria can enter. Secondary bacterial infection may cause increased pain, swelling, warmth, pus, and systemic symptoms.

Severe tinea capitis can cause inflammatory destruction of hair follicles and potentially permanent scarring alopecia.

Chronic fungal nail infection can cause significant nail deformity and may become a persistent source of fungal spread to surrounding skin.

Invasive fungal infections can cause organ damage, bloodstream infection, neurological complications, respiratory failure, and death, particularly in severely immunocompromised patients.

Prevention

Prevention of fungal infections involves reducing exposure, controlling moisture, maintaining skin integrity, and addressing underlying risk factors.

Keeping the skin clean and dry is particularly important in areas prone to fungal growth. After bathing, skin folds and spaces between the toes should be dried carefully.

Personal items such as towels, socks, shoes, combs, and nail equipment should not be shared when fungal infection is suspected.

Footwear should allow adequate ventilation, and damp socks should be changed promptly.

People using communal showers or swimming facilities may reduce transmission risk by avoiding barefoot contact with potentially contaminated surfaces.

Pets with suspected fungal skin disease should be evaluated by a veterinarian because untreated animal infection can contribute to recurrent human infection.

Patients with diabetes should maintain appropriate glucose control because metabolic abnormalities can increase susceptibility to infection.

People using inhaled corticosteroids can reduce the risk of oral candidiasis by following proper inhaler technique and rinsing the mouth after corticosteroid inhalation when advised.

When Medical Evaluation Is Important

A mild localized fungal rash may sometimes respond to appropriate topical treatment, but medical evaluation becomes particularly important when the diagnosis is uncertain or symptoms persist.

Medical assessment is advisable when an infection is widespread, rapidly worsening, recurrent, painful, associated with significant inflammation, or located in a high-risk anatomical site.

Scalp infection, extensive nail disease, recurrent genital symptoms, suspected invasive infection, and fungal disease in immunocompromised individuals generally warrant professional evaluation.

Fever, shortness of breath, coughing blood, confusion, severe weakness, low blood pressure, or other systemic symptoms in a person at risk for invasive fungal disease require urgent medical attention.

A persistent rash should not automatically be assumed to be fungal. Eczema, psoriasis, bacterial infection, allergic dermatitis, autoimmune disorders, and other conditions can resemble fungal disease.

Fungal Infections In Immunocompromised Patients

Immunocompromised patients require special consideration because infections that are mild in healthy individuals can become invasive in patients with impaired immune defenses.

Neutropenia, transplantation, chemotherapy, advanced HIV infection, prolonged corticosteroid use, and certain immune-modulating therapies can substantially increase susceptibility.

Invasive fungal infections may initially produce nonspecific symptoms. A persistent fever may be one of the earliest clues, particularly when antibacterial treatment does not produce an adequate response.

The lungs are an important site of invasive fungal disease, but dissemination can occur through the bloodstream. Brain, kidneys, liver, spleen, skin, and other organs may become involved.
