Introduction

Scabies is a highly contagious parasitic skin infestation caused by the microscopic mite Sarcoptes scabiei var. hominis. The mite burrows into the superficial layer of human skin, where it survives, feeds, and lays eggs. The resulting immune reaction produces the characteristic intense itching and skin eruption of scabies. Although scabies is generally treatable, delayed diagnosis, incomplete treatment, failure to treat close contacts, and inadequate control of transmission can result in persistent infestation and recurrent outbreaks.

Scabies occurs throughout the world and affects people of all ages, socioeconomic groups, and ethnic backgrounds. It is particularly common in settings characterized by overcrowding, close interpersonal contact, limited access to health care, and warm tropical climates. The World Health Organization (WHO) estimates that more than 200 million people are affected by scabies at any given time, with more than 400 million cases occurring cumulatively each year. Scabies is also recognized by WHO as a neglected tropical disease because of its substantial burden, especially in disadvantaged populations.

The clinical importance of scabies extends beyond itching and an unpleasant rash. Persistent scratching can damage the skin barrier and allow bacteria to enter, producing secondary bacterial infections such as impetigo. In vulnerable populations, these infections may progress to abscesses, cellulitis, invasive bacterial disease, sepsis, and post-streptococcal complications involving the kidneys or heart. Scabies can also cause significant sleep disturbance, impaired concentration, school or workplace absenteeism, psychological distress, and social stigma.

Understanding scabies therefore requires more than recognizing an itchy rash. Effective management involves identifying the characteristic distribution of lesions, considering the history of contact with affected individuals, confirming the diagnosis when necessary, selecting an appropriate scabicidal treatment, treating household and close contacts, managing complications, and preventing reinfestation.

Definition

Scabies is a contagious parasitic infestation of the skin caused by the human itch mite Sarcoptes scabiei var. hominis. The female mite penetrates the stratum corneum and creates superficial burrows in which she deposits eggs. The host develops an inflammatory and hypersensitivity response to the mite, its eggs, and its fecal material, producing intense pruritus and a characteristic papular or nodular eruption.

The disease can occur in two major clinical patterns: ordinary or classic scabies and crusted scabies. Classic scabies generally involves a relatively small number of mites and produces severe itching with burrows and papules. Crusted scabies, previously called Norwegian scabies, is a severe form characterized by extensive crusting and scaling and an extremely high mite burden. It occurs particularly in people with impaired cellular immunity or reduced ability to scratch and clear mites mechanically.

Scabies should be distinguished from conditions that merely resemble it clinically. Eczema, allergic dermatitis, insect bites, papular urticaria, drug eruptions, folliculitis, impetigo, and several other dermatological diseases may produce pruritic lesions. The combination of intense nocturnal itching, characteristic burrows, involvement of typical body sites, and similar symptoms among household or close contacts strongly supports scabies.

Causative Organism

The causative organism is Sarcoptes scabiei var. hominis, a microscopic ectoparasitic mite adapted to humans. It belongs to the phylum Arthropoda and class Arachnida. Adult mites are extremely small and cannot usually be seen with the naked eye.

The adult female is particularly important in human disease because she burrows into the superficial epidermis and lays eggs. After mating, the female mite establishes a burrow within the stratum corneum. She moves through the superficial skin layers while depositing eggs and fecal material. The male mite has a much shorter lifespan and generally remains on or near the skin surface.

The mite does not penetrate deeply into living tissue. Instead, it remains within the outer keratinized layer of the epidermis. Nevertheless, the host's immune response to mite proteins, eggs, and feces produces considerable inflammation and itching.

In ordinary scabies, the number of mites on an individual is usually relatively small. WHO notes that many affected individuals have approximately 10–15 mites. The situation is dramatically different in crusted scabies, where thousands or even millions of mites may be present. This enormous mite burden makes crusted scabies exceptionally contagious.

Life Cycle of the Scabies Mite

The life cycle begins when a fertilized female mite establishes a burrow within the superficial epidermis. The female lays eggs along the burrow as she moves through the stratum corneum.

The eggs generally hatch within several days. The emerging larvae subsequently develop through nymphal stages before becoming adult mites. Under suitable conditions, the developmental process from egg to adult may occur within approximately 1–2 weeks. The adult mites then reproduce, allowing the infestation to continue.

The mite's life cycle explains an important aspect of treatment: many scabicidal treatments are highly effective against active mites but may have limited activity against eggs. Consequently, treatment may need to be repeated according to the specific medication regimen so that newly hatched mites are eliminated before they mature and reproduce. WHO emphasizes that treatment regimens should account for this characteristic of the parasite's life cycle.

The mites depend heavily on human skin for survival. Away from the human host, ordinary scabies mites generally survive for only a limited period, commonly around 2–3 days. This is why environmental control focuses primarily on clothing, towels, bedding, and other items that have recently been in close contact with an affected person's skin rather than attempting to sterilize an entire home.

Epidemiology

Scabies is a worldwide disease, but its prevalence is particularly high in tropical and subtropical regions and in communities where overcrowding and limited access to treatment coexist. Children, older adults, institutionalized individuals, and people living in crowded environments are among the groups that may experience a high burden.

Transmission is facilitated by prolonged close skin-to-skin contact. Consequently, households, residential institutions, nursing facilities, shelters, boarding schools, refugee settlements, prisons, and other crowded environments can experience outbreaks.

The burden is especially significant in resource-limited settings because untreated infestation can persist within families and communities. Reinfestation may occur when only the symptomatic individual receives treatment while asymptomatic contacts remain untreated.

Scabies has important public-health consequences because it can facilitate secondary bacterial infections. WHO identifies impetigo as a frequent complication and notes associations between scabies-associated bacterial infections and serious complications such as sepsis, kidney disease, and rheumatic heart disease.

Risk Factors

The most important risk factor for scabies is close and prolonged physical contact with an infected person. Living in the same household is particularly important because family members frequently share beds, clothing, towels, and prolonged physical contact.

Household Crowding

Crowded living conditions increase the frequency and duration of skin-to-skin contact. When several people share sleeping spaces, scabies can spread rapidly from one person to another.

Institutional Living

Outbreaks can occur in nursing homes, hospitals, residential care facilities, prisons, shelters, boarding schools, and other institutions where individuals live or work in close proximity.

Contact With an Infected Person

A person may acquire scabies through prolonged direct skin contact with someone who is infested. Sexual contact is one possible form of close contact, but scabies is not exclusively a sexually transmitted infection. Children, family members, caregivers, and other household contacts can acquire it without any sexual exposure.

Crusted Scabies Exposure

Crusted scabies presents a particularly high transmission risk because the number of mites can be extraordinarily high. Even relatively limited contact with a person who has crusted scabies can result in transmission, and indirect environmental transmission becomes more relevant in this form.

Immunosuppression

People with impaired immune responses may be more likely to develop crusted scabies or experience atypical disease. Conditions or treatments that impair immunity can therefore alter the clinical presentation.

Older Age

Older adults may have atypical presentations and may develop extensive disease. Reduced mobility, institutional living, impaired sensation, or inability to scratch effectively can contribute to severe infestation.

Young Children

Infants and young children may develop lesions on areas that are less commonly involved in older children and adults, including the palms, soles, scalp, and sometimes the face.

Mode of Transmission

Scabies is primarily transmitted through prolonged, direct skin-to-skin contact with an infested person. The duration and intensity of contact matter because mites must transfer from one person's skin to another.

A brief handshake or casual social contact is generally much less likely to transmit ordinary scabies than prolonged physical contact. Close household contact, sharing a bed, prolonged caregiving, and intimate physical contact provide greater opportunities for transmission.

Indirect transmission through clothing, towels, bedding, and upholstered furniture can occur, although it is generally less important in ordinary scabies than direct contact. The importance of fomites increases considerably in crusted scabies because the affected person sheds very large numbers of mites.

An important epidemiological feature is the asymptomatic incubation period. A person who has recently acquired scabies may be able to transmit the mites before developing noticeable symptoms. WHO reports that symptoms in a person experiencing their first infestation usually develop several weeks after exposure. People who have had scabies previously may develop symptoms much sooner because of prior sensitization.

Pathogenesis

The pathogenesis of scabies begins with mechanical penetration of the skin by the female mite. The mite enters the superficial epidermis and creates a narrow burrow through the stratum corneum.

The mite does not normally invade the deeper dermis or subcutaneous tissue. Instead, it remains in the superficial keratinized layer while feeding and reproducing. Eggs and fecal pellets are deposited within the burrow.

The host immune system recognizes proteins and other substances associated with the mites and their products. This produces a delayed hypersensitivity and inflammatory response. The resulting inflammation contributes substantially to the papules, vesicles, erythema, and severe pruritus associated with scabies.

The itching may be disproportionately severe compared with the number of mites present. In ordinary scabies, an individual may have relatively few mites but experience intense generalized itching.

Scratching produces further mechanical damage to the epidermis. Repeated scratching disrupts the skin barrier and creates excoriations and erosions. These damaged areas provide an entry point for bacteria such as Staphylococcus aureus and Streptococcus pyogenes, resulting in secondary infection.

This relationship between mite infestation, inflammation, scratching, and bacterial infection explains why scabies can produce complications beyond the skin. WHO describes impetigo as a common complication and identifies potentially serious downstream complications such as sepsis and kidney disease.

Incubation Period

The incubation period varies depending on whether the person has experienced scabies previously.

In a person experiencing a first infestation, symptoms commonly appear several weeks after initial infestation because the immune system requires time to develop a clinically significant response. WHO generally describes symptom development around 4–6 weeks after infestation, while CDC materials describe a range of approximately 2–6 weeks.

During this period, the person may have few or no noticeable symptoms while still carrying mites and potentially transmitting them to close contacts.

In people who have had scabies previously, symptoms can develop much more rapidly following reinfestation because immune sensitization has already occurred. This may make recurrent cases appear to develop suddenly.

The prolonged asymptomatic period is one reason why scabies can spread silently through a household. When one person develops severe itching, other household members may already be infested even though they feel completely well.

Clinical Features

The clinical presentation of scabies is characterized primarily by intense pruritus and a characteristic distribution of skin lesions.

Pruritus

Severe itching is the hallmark symptom. It is often particularly intense at night and may interfere substantially with sleep.

Nocturnal itching occurs because of a combination of behavioral, environmental, and inflammatory factors, but the precise biological contribution of each factor varies. Clinically, however, nighttime worsening is a highly useful clue when evaluating a patient with a generalized itchy eruption.

The itching may become generalized even when the visible lesions are relatively localized.

Skin Lesions

Scabies may produce multiple types of lesions, including:

  • Small erythematous papules
  • Vesicles
  • Excoriations
  • Crusts
  • Nodules
  • Linear burrows
  • Secondary pustules or erosions caused by bacterial infection

The appearance may be altered significantly by scratching. Consequently, the lesions seen during examination may no longer resemble the original mite-induced lesions.

Burrows

Burrows are among the most characteristic findings of scabies. They appear as thin, irregular, thread-like or linear lesions within the superficial epidermis.

They may be easier to identify in certain locations, particularly the finger webs, wrists, and sides of the hands. A small papule or vesicle may be present at one end of a burrow where the female mite is located.

Burrows may be difficult to identify in heavily scratched or secondarily infected skin.

Typical Distribution of Lesions

The distribution of scabies lesions is extremely important diagnostically.

Commonly affected areas include:

  • Finger webs
  • Sides of the fingers
  • Wrists
  • Elbows
  • Axillary regions
  • Waistline
  • Abdomen
  • Buttocks
  • Genital region
  • Breasts, particularly around the nipples
  • Hands and feet

In adults, the head and neck are usually spared in classic scabies. However, this pattern is not universal, particularly in infants, young children, older adults, and people with crusted disease.

Genital involvement can be an important diagnostic clue. In adult males, itchy papules or nodules may occur on the penis and scrotum. In females, lesions may occur around the breasts.

Scabies in Infants and Young Children

Scabies may look substantially different in infants and young children than in adults.

Young children may develop a widespread eruption involving the palms and soles. The scalp may also be involved, particularly in infants. Lesions can be vesicular, papular, nodular, or crusted.

Because infants cannot describe itching reliably, irritability, disturbed sleep, feeding difficulties, and frequent scratching or rubbing may be important clinical clues.

The diagnosis can be challenging because many pediatric dermatological conditions produce itchy rashes. A history of similar symptoms among household members can provide a major diagnostic clue.

Treatment in very young children requires particular attention to the medication chosen, the formulation, and the instructions for application. Medication should be used according to age-specific guidance and under appropriate clinical supervision.

Scabies in Older Adults

Older adults may develop atypical or extensive scabies. The distribution may be less classic, and itching may not always be as prominent as expected.

Institutionalized older adults are particularly vulnerable to outbreaks because of close contact with caregivers and other residents. Mobility limitations, cognitive impairment, and difficulty communicating symptoms can delay recognition.

When a patient in a residential facility develops unexplained generalized pruritus, scabies should remain in the differential diagnosis, especially if multiple residents or caregivers develop itching.

Nodular Scabies

Nodular scabies is characterized by persistent, intensely itchy inflammatory nodules. They may occur particularly on the genitalia, groin, buttocks, and other areas.

These nodules can persist after successful eradication of the mites because the immune response may continue even after the infestation has been eliminated.

This creates an important clinical distinction: persistent itching or nodules after treatment do not automatically mean treatment failure. The clinician should assess whether there are new burrows, new lesions, continuing exposure to untreated contacts, or evidence of reinfestation before deciding that additional scabicidal treatment is necessary.

Crusted Scabies

Crusted scabies is the most severe clinical form of the disease. It is characterized by extensive hyperkeratotic, crusted, scaly plaques that may involve large portions of the body.

Unlike classic scabies, crusted scabies can contain enormous numbers of mites. WHO notes that affected individuals may have thousands or millions of mites. Because of this extremely high mite burden, crusted scabies is highly contagious.

Crusted scabies is particularly associated with impaired immune responses, although it can also occur in people without a clearly recognized immunocompromising condition.

An important diagnostic pitfall is that itching may be less prominent than expected. A person with extensive crusted lesions may not complain of severe pruritus, which can delay diagnosis.

Crusted scabies can involve the hands, feet, scalp, trunk, and other areas. The nails may become thickened or dystrophic, and subungual infestation may contribute to persistent infection.

Because of the massive mite burden, crusted scabies can generate widespread environmental contamination and cause outbreaks in households, hospitals, and residential institutions.

Management generally requires more intensive treatment than classic scabies and often involves both topical and systemic therapy. Close contacts should be identified and managed promptly.

Clinical Features Suggesting Scabies

Several findings together strongly suggest scabies:

  1. Severe generalized itching, particularly at night.
  2. Itchy papules with a characteristic distribution.
  3. Burrows in the finger webs or wrists.
  4. Genital or nipple involvement.
  5. Similar itching among household members.
  6. A history of prolonged close contact with an affected person.
  7. Persistent itching despite treatment for eczema or another presumed dermatological disorder.
  8. Crusted or hyperkeratotic lesions in an immunocompromised or institutionalized patient.

No single feature is perfectly diagnostic. The diagnosis is made by integrating the history, examination, lesion distribution, epidemiological context, and, when necessary, microscopic or dermatoscopic findings.

Diagnosis

The diagnosis of scabies is primarily clinical. A clinician should examine the entire skin surface rather than focusing only on the area where the patient reports itching.

Careful inspection of the finger webs, wrists, elbows, axillae, waistline, buttocks, genital region, breasts, palms, and soles can reveal characteristic lesions.

In children, the scalp and other areas should also be examined.

When typical lesions are present in a patient with intense nocturnal pruritus and affected household contacts, laboratory confirmation may not be necessary. WHO states that clinical recognition is usually sufficient, particularly in settings where scabies is common.

However, diagnostic testing can be useful when the clinical presentation is atypical, when the diagnosis is uncertain, or when confirmation is important during an institutional outbreak.

Skin Scraping and Microscopy

Skin scraping can be used to search for mites, eggs, or fecal pellets.

The clinician selects a suspicious lesion, preferably an intact burrow or characteristic papule, and obtains superficial material from the affected area. The specimen is then examined microscopically.

Finding the mite, egg, or fecal material provides strong evidence of infestation.

However, a negative scraping does not necessarily exclude scabies. The infestation may be sparse, the wrong lesion may have been sampled, or the specimen may not contain a mite or its products.

Therefore, microscopy should be interpreted in conjunction with the clinical picture.

Dermoscopy

Dermoscopy can assist in diagnosing scabies by allowing magnified visualization of characteristic structures within the skin.

A commonly described appearance is a small triangular or dark structure at the end of a burrow, representing the mite, with a surrounding linear burrow.

Dermoscopy can be particularly useful when burrows are difficult to identify by routine inspection. It is noninvasive and can improve diagnostic confidence when performed by a clinician familiar with the technique.

WHO recognizes dermatoscopy and microscopy as useful supportive diagnostic approaches but emphasizes that they are not always necessary for routine diagnosis.

Differential Diagnosis

Scabies has a broad differential diagnosis because many conditions cause itching and papular eruptions.

Atopic Dermatitis

Atopic dermatitis commonly causes chronic pruritus, xerosis, erythematous plaques, and excoriations. However, the presence of burrows, genital lesions, nocturnal predominance, and affected household contacts favors scabies.

Allergic Contact Dermatitis

Contact dermatitis may produce intensely itchy erythematous or vesicular lesions corresponding to an exposure pattern. The distribution and exposure history usually provide clues.

Urticaria

Urticaria produces transient wheals that typically migrate and disappear within hours. Individual urticarial lesions do not usually remain fixed for days and do not produce burrows.

Insect Bites

Bedbugs, fleas, and other arthropods can cause itchy papules. Exposure history, lesion distribution, and examination of the environment may help distinguish these conditions.

Papular Urticaria

Papular urticaria is a hypersensitivity reaction to insect bites, particularly in children. It can resemble scabies but generally lacks characteristic burrows and household clustering typical of scabies.

Folliculitis

Folliculitis causes inflammatory lesions centered on hair follicles and may be itchy or painful. The distribution and follicular nature of the lesions distinguish it from typical scabies.

Impetigo

Impetigo can occur as a complication of scabies but may also occur independently. Honey-colored crusts and superficial erosions suggest impetigo, while the presence of intense nocturnal pruritus and burrows indicates underlying scabies.

Drug Eruption

Generalized drug eruptions may produce widespread pruritic rashes. A medication history and absence of characteristic burrows help distinguish drug reactions from scabies.

Complications

Although scabies itself is usually treatable, complications can be clinically important.

Secondary Bacterial Infection

The most common important complication is bacterial superinfection resulting from scratching and disruption of the skin barrier.

Common organisms include Staphylococcus aureus and Streptococcus pyogenes. Infection may present with increasing redness, warmth, swelling, tenderness, pustules, purulent discharge, or crusting.

Impetigo

Impetigo is a superficial bacterial infection characterized by erosions and crusting. Scabies-associated impetigo is particularly important in children and in tropical or resource-limited settings.

Cellulitis and Abscess

Bacterial infection can extend into deeper tissues and cause cellulitis or abscess formation.

Sepsis

Severe bacterial infection may progress to bloodstream infection and sepsis, particularly in vulnerable patients.

Post-Streptococcal Glomerulonephritis

Scabies-associated S. pyogenes skin infection has been associated with acute post-streptococcal glomerulonephritis. WHO identifies kidney complications among the important consequences of scabies-associated bacterial infection.

Rheumatic Heart Disease

Repeated streptococcal infections in communities with high scabies and impetigo burdens may contribute to the broader burden of rheumatic heart disease. This association is particularly relevant in resource-limited tropical settings.

Sleep Disturbance

Persistent nocturnal itching may produce substantial sleep deprivation. This can impair concentration, school performance, work productivity, mood, and overall quality of life.

Psychological and Social Effects

Visible skin lesions and the misconception that scabies results from poor hygiene can produce embarrassment and stigma. Patients may avoid social contact or experience anxiety about transmitting the condition to others.

Treatment Principles

Successful treatment of scabies requires more than applying a medication to visible lesions. The objective is to eliminate living mites, prevent newly hatched mites from continuing the life cycle, treat close contacts when indicated, manage complications, and prevent reinfestation.

The choice of treatment depends on the patient's age, pregnancy status, body weight, clinical severity, immune status, presence of crusted disease, local availability of medications, and clinician or national guideline recommendations.

WHO identifies several topical scabicides, including 5% permethrin, malathion, benzyl benzoate, and sulfur preparations. Oral ivermectin is also highly effective for appropriate patients, but important restrictions apply to certain populations.

Permethrin

Permethrin 5% cream is one of the principal treatments for classic scabies.

It is generally applied to the skin according to product and clinical guidance, with careful attention to areas that are commonly missed, such as the finger webs, under the nails, wrists, genital region, buttocks, and skin folds.

Treatment must be performed correctly. Inadequate application is a common reason for apparent treatment failure.

Depending on the regimen and local guidance, treatment may be repeated after approximately one week. WHO notes that repeated treatment is often necessary because scabicides may not eliminate all eggs.

In young children, application instructions differ from those for adults because the scalp and other areas may be involved.

Patients should follow the instructions provided with the specific product and their clinician's recommendations rather than assuming that every topical preparation should be applied in exactly the same manner.

Oral Ivermectin

Ivermectin is an oral antiparasitic medication that is highly effective against scabies in appropriately selected patients.

A commonly used regimen for classic scabies is weight-based dosing, with repeat dosing according to clinical guidance because ivermectin does not reliably eliminate scabies eggs.

WHO notes that ivermectin should not be used in pregnant women or children weighing less than 15 kg because safety has not been established adequately in these groups.

Ivermectin is particularly useful when topical treatment is impractical, when there is extensive disease, during outbreaks, or when topical therapy has failed or cannot be used. However, the exact regimen should be selected by a qualified clinician.

Treatment of Crusted Scabies

Crusted scabies requires substantially more aggressive management than classic scabies because of the enormous number of mites.

Treatment generally requires a combination of systemic and topical therapy, repeated according to clinical response and the chosen regimen. WHO specifically emphasizes that patients with crusted scabies require intensive treatment with both topical and oral medications.

Treatment of crusted disease should also address the thick crust and hyperkeratosis because these can prevent topical medications from reaching the mites effectively.

Patients with crusted scabies should be managed carefully because they are highly contagious and can serve as a major source of transmission to family members, healthcare workers, and other residents in institutional environments.

Treatment Failure

Persistent itching after treatment does not necessarily indicate that the scabies mites are still alive.

Pruritus can continue for 1–2 weeks or longer after successful treatment because the immune response to mite antigens may persist. WHO specifically advises that itching can intensify temporarily after effective treatment.

Treatment failure or reinfestation should be considered when:

  • New burrows appear after treatment.
  • New papules continue to develop.
  • Close contacts were not treated.
  • Treatment was not applied correctly.
  • Treatment was washed off too early.
  • Some body areas were missed.
  • A second treatment was required but not performed.
  • The patient was re-exposed to an untreated person.
  • The diagnosis was incorrect.
  • Crusted scabies was inadequately treated.

The clinician should distinguish persistent post-scabetic itching from active infestation before repeatedly administering scabicides.

Treatment of Household Contacts

Treating only the symptomatic patient is one of the most common reasons for recurrence.

Household members and other close contacts may be infested without symptoms because of the delay between infestation and the development of itching.

For this reason, WHO recommends treating household members simultaneously when one member has scabies, according to the appropriate treatment strategy.

The exact definition of contacts requiring treatment can depend on the clinical circumstances, type of exposure, and local public-health guidance.

Close contacts should receive clear instructions regarding medication application, laundering of recently used clothing and bedding, and avoidance of prolonged skin-to-skin contact until treatment has been completed as advised.

Environmental Control

Environmental control is an important component of scabies management, but excessive environmental disinfection is usually unnecessary for ordinary scabies.

Mites generally survive away from human skin for only a limited period. Therefore, attention should be directed toward items that have had recent prolonged contact with the affected person's skin.

Clothing, bedding, and towels should be washed and dried appropriately. WHO recommends hot washing and hot drying where possible, while items that cannot be washed can be sealed in a plastic bag for several days.

CDC similarly recommends decontaminating clothing, bedding, and towels used by affected individuals and close contacts, using hot washing and drying or dry cleaning, with non-washable items sealed in a plastic bag for at least 72 hours.

Routine household cleaning is generally sufficient for ordinary scabies. More extensive environmental measures may be appropriate in crusted scabies because of the much greater mite burden.

Prevention

Prevention depends primarily on early recognition, prompt treatment, simultaneous management of appropriate contacts, and interruption of transmission.

People with suspected scabies should minimize prolonged skin-to-skin contact with others until treatment has been completed according to medical advice.

Household members should be informed that they may be infested even if they have no symptoms.
