Introduction

Upper respiratory tract infection (URTI) refers to an infection involving one or more structures of the upper respiratory system, including the nose, nasal cavity, paranasal sinuses, pharynx, tonsils, and larynx. URTIs are among the most common infectious illnesses encountered in primary care and community settings. They occur in people of all ages, but children, older adults, individuals with chronic diseases, and people with weakened immune systems may experience more frequent or severe infections.

The term URTI describes a broad group of conditions rather than one specific disease. The common cold, acute viral rhinitis, acute pharyngitis, tonsillitis, laryngitis, and many cases of acute sinusitis fall within this spectrum. Although these conditions may produce similar symptoms, their causes, clinical presentations, complications, and treatment requirements can differ.

Most uncomplicated URTIs are caused by viruses and are self-limiting. Common respiratory viruses can produce nasal congestion, rhinorrhea, sneezing, sore throat, cough, headache, malaise, and sometimes fever. Antibiotics do not treat viral infections and therefore are not routinely indicated for uncomplicated viral URTIs. NICE guidance similarly emphasizes that acute cough associated with a URTI is usually viral and generally does not require antibiotics.

Understanding URTIs is important not only for recognizing and treating illness but also for preventing unnecessary antibiotic use. Respiratory symptoms can be uncomfortable and may interfere significantly with sleep, work, education, appetite, and daily activities. At the same time, symptoms such as persistent high fever, difficulty breathing, severe dehydration, altered mental status, severe unilateral facial pain, or rapidly worsening illness may indicate a complication or an alternative diagnosis requiring medical evaluation.

Anatomy of the Upper Respiratory Tract

The upper respiratory tract forms the initial pathway through which inhaled air enters the body. It also performs important protective functions, including filtration, humidification, warming of inspired air, mucociliary clearance, and immune defense.

The nose and nasal cavity represent the first major components of the upper respiratory tract. Nasal hairs and mucus trap dust, microorganisms, and other particles. The nasal mucosa contains a rich vascular supply that helps warm inhaled air, while mucus helps humidify it. Ciliated epithelial cells move mucus toward the nasopharynx, where it can be swallowed or expelled.

Behind the nasal cavity lies the pharynx, which is divided into the nasopharynx, oropharynx, and laryngopharynx. The pharynx serves as a common passage for respiratory and gastrointestinal systems. Infection of different portions of the pharynx can produce different clinical patterns, ranging from mild throat irritation to severe pharyngitis and tonsillitis.

The tonsils are collections of lymphoid tissue positioned strategically around the entrance to the respiratory and digestive tracts. They contribute to immune surveillance by encountering microorganisms entering through the mouth and nose. Inflammation of the tonsils may occur as part of a viral infection or, in some cases, as a bacterial infection.

The paranasal sinuses are air-filled cavities within the skull that communicate with the nasal cavity. They include the frontal, maxillary, ethmoid, and sphenoid sinuses. Viral respiratory infections can cause inflammation and obstruction of sinus drainage pathways, producing congestion, facial pressure, and nasal discharge. Most acute sinus symptoms, particularly early in the illness, are viral in origin.

The larynx connects the pharynx with the lower respiratory tract and contains the vocal cords. Inflammation of the larynx can produce hoarseness, voice changes, throat discomfort, and sometimes a characteristic barking cough in children when associated with viral croup.

What Causes Upper Respiratory Tract Infections?

Viruses are responsible for the majority of uncomplicated URTIs. Numerous viruses can infect the respiratory epithelium, and different viruses may produce overlapping clinical syndromes.

Rhinoviruses are among the most important causes of the common cold. Other viral causes include seasonal coronaviruses, respiratory syncytial virus (RSV), adenoviruses, parainfluenza viruses, influenza viruses, and human metapneumovirus. SARS-CoV-2 can also produce an upper respiratory illness, particularly in individuals with mild disease.

The exact clinical presentation depends on the virus, the patient's age, previous immunity, underlying health, and the site of infection.

Bacteria can also cause infections involving the upper respiratory tract, but they account for a smaller proportion of uncomplicated respiratory illnesses. Important bacterial conditions include group A streptococcal pharyngitis and selected cases of bacterial sinusitis. Bacterial infection should therefore be considered based on the clinical pattern rather than simply because symptoms are severe or because nasal mucus has become colored.

Antibiotics are effective against susceptible bacteria but do not treat viruses. Unnecessary antibiotic treatment exposes patients to adverse effects and contributes to antimicrobial resistance. The CDC specifically emphasizes that antibiotics do not work against the viruses responsible for common colds.

How Upper Respiratory Infections Spread

URTIs are highly contagious because many respiratory viruses are efficiently transmitted between people.

Transmission can occur through respiratory droplets and particles released when an infected person coughs, sneezes, talks, or breathes. Transmission may also occur when contaminated hands or surfaces come into contact with the nose, mouth, or eyes.

Crowded environments increase opportunities for transmission. Schools, daycare centers, workplaces, public transportation, healthcare facilities, and households can all facilitate the spread of respiratory viruses.

A person may transmit infection before realizing that they are ill. This makes prevention challenging, particularly for viruses that produce mild symptoms.

Hand hygiene, respiratory etiquette, improved ventilation, staying home when acutely ill, avoiding close contact with vulnerable individuals during periods of infectiousness, and appropriate vaccination against vaccine-preventable respiratory diseases can reduce transmission.

Risk Factors

Although anyone can develop a URTI, certain circumstances increase susceptibility.

Young children experience frequent respiratory infections because their immune systems are still developing and they have repeated exposure to respiratory pathogens in daycare and school environments. Close contact with other children also facilitates transmission.

Adults who work in schools, hospitals, daycare centers, crowded workplaces, or other settings involving frequent interpersonal contact may also have increased exposure.

Smoking and exposure to tobacco smoke can impair normal respiratory defense mechanisms. Tobacco smoke damages airway epithelium and interferes with mucociliary clearance, making respiratory infections more likely and potentially more severe.

Poor sleep, significant physical stress, inadequate nutrition, and certain chronic medical conditions may also influence susceptibility or recovery.

People with asthma, chronic lung disease, cardiovascular disease, diabetes, kidney disease, or immunosuppression may be at increased risk of complications from respiratory infections. Older adults and very young children may also require closer monitoring when symptoms are significant.

Common Types of Upper Respiratory Tract Infections

URTIs encompass several clinical conditions.

The common cold is the classic example. It usually produces nasal congestion, rhinorrhea, sneezing, sore throat, cough, and mild systemic symptoms.

Acute viral pharyngitis primarily affects the throat and may cause sore throat, discomfort during swallowing, mild fever, cough, and generalized malaise.

Tonsillitis involves inflammation of the tonsils. It may be viral or bacterial and can produce swollen tonsils, throat pain, fever, and difficulty swallowing.

Acute rhinitis causes inflammation of the nasal mucosa and typically results in sneezing, nasal obstruction, and watery or mucoid nasal discharge.

Acute sinusitis involves inflammation of the paranasal sinus mucosa. It commonly follows a viral upper respiratory infection.

Laryngitis affects the larynx and vocal cords and commonly causes hoarseness or loss of voice.

In children, certain viral infections can cause croup, characterized by inflammation around the larynx and upper airway with a barking cough and sometimes inspiratory stridor.

Pathophysiology of URTI

The pathophysiology begins when an infectious organism reaches the mucosal surfaces of the upper respiratory tract.

Respiratory viruses typically attach to receptors on epithelial cells and enter susceptible cells. Viral replication triggers local cellular injury and an inflammatory response.

The immune system responds by activating innate immune mechanisms. Inflammatory mediators increase blood flow and vascular permeability within the mucosa. This contributes to swelling and congestion.

Increased mucus production is another important response. Mucus traps microorganisms and particles, while ciliary movement attempts to transport the material toward the pharynx.

The combination of mucosal edema, increased secretions, and inflammatory mediator release produces many familiar symptoms. Nasal congestion occurs because swollen mucosal tissues narrow the nasal passages. Rhinorrhea results from increased glandular secretion and vascular leakage.

Sneezing is a protective reflex designed to remove irritants and secretions from the nasal passages.

Sore throat develops because inflammatory processes affect the pharyngeal mucosa and sensory nerve endings.

Cough may develop because secretions drain posteriorly into the pharynx and because inflammatory stimulation activates cough receptors.

Systemic symptoms such as fatigue, headache, chills, and mild fever can result from cytokine-mediated immune responses.

The clinical manifestations therefore represent a combination of direct pathogen effects, local tissue inflammation, and the body's immune response.

Clinical Features

The clinical presentation of a URTI varies according to the specific infection and the individual patient.

Nasal symptoms are among the most common features. Patients may develop sneezing, nasal itching, congestion, rhinorrhea, and postnasal drainage.

The nasal discharge is often initially clear and watery. As inflammation progresses, mucus may become thicker and appear white, yellow, or green. Importantly, the color of nasal mucus alone does not establish a bacterial infection.

Sore throat is another frequent complaint. It may range from mild irritation to significant pain during swallowing.

Cough can occur because of postnasal drainage, pharyngeal irritation, or inflammation of the upper airway. An acute cough associated with a respiratory infection may persist after other symptoms have improved. NICE notes that acute cough may last up to three or four weeks, even when the underlying infection is self-limiting.

Fever is variable. Adults with uncomplicated common colds may have no fever or only a low-grade temperature, whereas influenza and some other viral infections can produce more prominent fever and systemic symptoms.

Fatigue and malaise are common. Some patients also experience headache, reduced appetite, mild muscle aches, or a sensation of pressure around the face.

Symptoms in Children

Children commonly experience URTIs and may develop symptoms that differ somewhat from those seen in adults.

A young child may present with irritability, reduced feeding, disturbed sleep, rhinorrhea, cough, fever, or increased crying. Infants may have difficulty feeding because nasal obstruction interferes with breathing during feeding.

Older children can describe sore throat, headache, ear discomfort, facial pressure, or difficulty swallowing.

Because children may not always communicate symptoms accurately, caregivers should pay attention to behavioral changes, hydration, respiratory effort, urine output, and overall activity level.

Certain complications, including otitis media, are more common in children because of anatomical and developmental factors.

Symptoms in Adults

Adults with uncomplicated viral URTI often develop a combination of nasal congestion, runny nose, sneezing, sore throat, cough, headache, and fatigue.

Some individuals have minimal systemic symptoms, whereas others experience substantial tiredness and malaise.

Influenza can produce a more abrupt and systemic illness, often characterized by fever, chills, headache, myalgia, and pronounced fatigue. COVID-19 and other respiratory viral infections can also present with overlapping symptoms.

Because symptoms overlap considerably, clinical history, examination, local epidemiology, and appropriate testing may be needed when identifying a specific viral illness is important for treatment or infection-control decisions.

Common Cold

The common cold is one of the most frequent manifestations of an upper respiratory viral infection.

It usually begins gradually with throat irritation, sneezing, nasal congestion, and rhinorrhea. Cough may develop during the illness or become more prominent later.

Symptoms generally improve spontaneously. However, the duration can vary depending on the virus and individual factors.

Patients sometimes interpret persistent cough or colored mucus as evidence that the infection has become bacterial. This is not necessarily the case. Viral inflammation can continue after the initial infection has begun to resolve, and mucus characteristics may change during the normal course of illness.

Treatment is therefore primarily supportive unless a specific complication or alternative diagnosis is identified.

Acute Pharyngitis and Tonsillitis

Acute pharyngitis refers to inflammation of the pharynx and is commonly caused by viruses.

Patients may complain of sore throat, pain when swallowing, fever, headache, and generalized discomfort. Cough and rhinorrhea favor a viral cause.

Group A Streptococcus is an important bacterial cause of pharyngitis, particularly in children and adolescents. Clinical features can include sudden sore throat, fever, tender anterior cervical lymph nodes, tonsillar inflammation, and absence of cough.

Because bacterial and viral pharyngitis can overlap clinically, selected patients may require a rapid antigen detection test or throat culture depending on age, clinical findings, and local recommendations.

Confirmed streptococcal infection may warrant antibiotic treatment to reduce complications and transmission.

Acute Sinusitis

Acute sinusitis, often called acute rhinosinusitis, commonly develops after a viral upper respiratory infection.

Inflammation causes swelling of the sinus openings and can impair normal drainage. Accumulated secretions and mucosal edema produce nasal obstruction and facial pressure.

Symptoms may include nasal congestion, nasal discharge, facial pressure or pain, reduction in smell, headache, cough, and postnasal drainage.

Most early acute sinus infections are viral. NICE guidance states that antibiotics should not routinely be offered when sinusitis symptoms have been present for around 10 days or less without features suggesting a more serious or bacterial course.

Bacterial sinusitis becomes more likely when symptoms persist without improvement, become significantly worse after initial improvement, or are accompanied by a pattern of severe illness. Clinical assessment is important before deciding whether antibiotic treatment is appropriate.

Laryngitis

Laryngitis is inflammation of the larynx and vocal cords.

The most characteristic symptom is hoarseness. Patients may also experience throat irritation, dry cough, discomfort while speaking, or temporary loss of voice.

Viral infections are a common cause. Excessive voice use can worsen symptoms and delay recovery.

Voice rest, adequate hydration, and treatment of associated respiratory symptoms are generally important. Persistent hoarseness should not automatically be attributed to an acute infection and may require evaluation for other causes, particularly when it lasts longer than expected.

Diagnosis of Upper Respiratory Tract Infection

The diagnosis of an uncomplicated URTI is usually clinical.

A detailed history should establish the onset, duration, progression, and severity of symptoms. Important questions include whether the patient has fever, cough, nasal discharge, sore throat, facial pain, shortness of breath, chest pain, difficulty swallowing, or altered mental status.

The clinician should also ask about exposure to sick contacts, recent travel when relevant, vaccination status, smoking, underlying illnesses, medications, allergies, and previous episodes.

Physical examination should assess general appearance, temperature, respiratory rate, oxygen saturation when clinically indicated, nasal mucosa, pharynx, tonsils, cervical lymph nodes, ears, and chest.

Most uncomplicated cases do not require extensive laboratory testing.

When Testing Is Useful

Testing should be guided by the clinical situation rather than performed routinely for every person with cold symptoms.

Testing for influenza or COVID-19 may be useful when the result could affect treatment, infection-control decisions, or management of individuals at higher risk of severe disease.

Patients with suspected streptococcal pharyngitis may require throat testing when clinical features suggest bacterial infection.

Additional investigations may be necessary if symptoms are severe, prolonged, recurrent, atypical, or associated with signs of complications.

Chest imaging is generally unnecessary for a simple URTI but may be considered when pneumonia or another lower respiratory condition is suspected.

Differential Diagnosis

Several conditions can resemble an uncomplicated URTI.

Allergic rhinitis may produce sneezing, nasal congestion, and rhinorrhea but usually lacks the systemic symptoms of an acute infection. Itching of the eyes and nose and recurrent exposure-related symptoms may suggest an allergic cause.

Influenza often produces a more abrupt onset and more prominent systemic symptoms than the typical common cold.

COVID-19 can present with upper respiratory symptoms and may be clinically indistinguishable from other viral infections without testing.

Pneumonia should be considered when there is significant breathlessness, hypoxia, pleuritic chest pain, persistent high fever, confusion, or abnormal chest findings.

Asthma exacerbation may be triggered by respiratory infections and is suggested by wheezing, chest tightness, and increased work of breathing.

Gastroesophageal reflux, postnasal drainage, smoking, and other conditions may contribute to persistent cough after the acute infection has resolved.

Treatment Principles

Treatment of an uncomplicated viral URTI is primarily supportive.

The main objectives are to maintain hydration, relieve bothersome symptoms, support adequate rest, and identify patients who require further evaluation.

Adequate fluid intake may help prevent dehydration, particularly in patients with fever, reduced oral intake, vomiting, or increased respiratory losses.

Rest is useful when patients feel fatigued, although strict bed rest is not required for every uncomplicated infection.

Warm fluids may provide temporary relief of throat irritation. Honey can help relieve cough in appropriate age groups, but honey should not be given to infants younger than one year because of the risk of infant botulism.

Saline nasal drops or sprays may help relieve nasal congestion and clear secretions.

Humidified air may provide subjective comfort for some patients, although excessively hot steam should be avoided because of burn risk.

Management of Fever and Pain

Paracetamol (acetaminophen) or ibuprofen may be used for fever, headache, sore throat, and generalized discomfort when appropriate for the individual patient.

Medication selection should take into account age, pregnancy status, kidney disease, liver disease, gastrointestinal disease, anticoagulant use, allergies, and other medications.

Patients should be cautious with combination cold preparations because several products may contain the same active ingredient. Accidental duplication can lead to excessive dosing.

Children should receive age- and weight-appropriate medication dosing. Aspirin should not be given to children or adolescents with viral illnesses because of the association with Reye syndrome.

Nasal Congestion Management

Nasal congestion can be particularly troublesome because it interferes with sleep and normal breathing through the nose.

Saline irrigation or saline sprays are generally safe options for symptomatic relief when used correctly.

Topical nasal decongestants can provide short-term relief but should not be used continuously for prolonged periods because excessive use can result in rebound nasal congestion.

Oral decongestants may not be suitable for everyone. Individuals with certain cardiovascular conditions, uncontrolled hypertension, glaucoma, prostate-related urinary symptoms, or other contraindications should seek professional advice before using them.

In children, particularly young children, many over-the-counter cold preparations have limited usefulness and may carry safety concerns. Medication should therefore be selected according to age and local guidance.

Management of Sore Throat

Sore throat can often be managed with fluids, warm beverages, saline gargles in appropriate patients, and simple analgesics.

Salt-water gargling may provide temporary symptomatic relief in older children and adults who can gargle safely without swallowing the solution.

Throat lozenges can reduce discomfort in some patients, although they are inappropriate for young children because of choking risk.

If streptococcal pharyngitis is confirmed or strongly suspected according to clinical and testing criteria, antibiotic treatment may be appropriate.

The key principle is that antibiotics should be directed toward confirmed or appropriately suspected bacterial infection rather than used automatically for every sore throat.

Management of Cough

Cough is a protective mechanism that helps clear secretions and irritants from the respiratory tract.

An acute cough associated with a URTI may continue after nasal and throat symptoms have begun to improve. This does not necessarily indicate treatment failure or bacterial infection.

Adequate hydration can help maintain comfortable mucus clearance.

Honey can reduce cough severity in children over one year of age and adults. Warm fluids may also soothe throat irritation.

Over-the-counter cough medications have variable effectiveness, and some are inappropriate for young children. Their use should therefore be guided by age, symptoms, contraindications, and local recommendations.

NICE advises that antibiotics should not routinely be offered for acute cough associated with an upper respiratory infection because the condition is usually viral and self-limiting.

Antibiotics: When Are They Actually Needed?

One of the most important principles in URTI management is distinguishing viral disease from bacterial infection.

Antibiotics do not kill viruses. Therefore, giving an antibiotic for an uncomplicated viral cold does not speed recovery and exposes the patient to potential adverse effects.

The CDC emphasizes that antibiotics are useful for certain bacterial infections but do not treat viral illnesses.

Unnecessary antibiotic use may cause nausea, diarrhea, allergic reactions, drug interactions, and other adverse effects. More importantly, excessive antibiotic exposure contributes to antimicrobial resistance.

Antibiotics may be appropriate when a bacterial diagnosis has been established or when clinical assessment indicates a significant likelihood of bacterial infection requiring treatment.

Examples can include confirmed group A streptococcal pharyngitis and selected cases of bacterial sinusitis.

The choice of antibiotic should depend on the specific diagnosis, patient age, allergy history, pregnancy status, local resistance patterns, and applicable clinical guidelines.

Patients should never use leftover antibiotics from previous illnesses or take antibiotics prescribed to another person.

Why Colored Mucus Does Not Automatically Mean Bacterial Infection

A common misconception is that yellow or green nasal discharge proves that a patient has a bacterial infection.

During an inflammatory response, immune cells, cellular debris, proteins, and mucus can alter the appearance of nasal secretions. Therefore, mucus may become yellow or green during a viral infection.

The decision to use antibiotics should be based on the overall clinical picture, duration and progression of illness, examination findings, and specific diagnostic criteria rather than mucus color alone.

This distinction is particularly important because the mistaken belief that colored mucus requires antibiotics can contribute substantially to inappropriate antibiotic prescribing.

Antiviral Treatment

Antiviral therapy is different from antibiotic therapy because it targets viruses rather than bacteria.

Specific antiviral drugs are available for certain respiratory viral infections, particularly influenza and COVID-19, and may be most beneficial when administered early and in patients who meet treatment criteria.

Testing can help guide clinical management when identifying influenza or SARS-CoV-2 would alter treatment decisions. CDC guidance notes that diagnostic testing for COVID-19 and influenza can help guide treatment, particularly in people at higher risk of progression to severe disease.

Antiviral treatment is not routinely required for every patient with mild upper respiratory symptoms. Eligibility depends on the pathogen, timing of illness, risk factors, severity, and current treatment recommendations.

Complications of Upper Respiratory Tract Infections

Most URTIs resolve without serious complications, but complications can occasionally develop.

Otitis media is particularly common in children following respiratory infections. Nasal and nasopharyngeal inflammation can interfere with normal Eustachian tube function and contribute to middle-ear fluid accumulation and infection.

Sinusitis may develop when inflammation persists and sinus drainage becomes impaired.

A viral infection can also trigger an exacerbation of asthma or chronic airway disease.

More serious complications are uncommon but can occur. Severe pharyngitis may rarely progress to deep neck space infection or peritonsillar abscess.

Lower respiratory tract complications can include acute bronchitis and pneumonia. The appearance of significant breathlessness, chest pain, hypoxia, persistent high fever, or marked systemic deterioration should prompt reassessment.

Red Flag Symptoms

Although most URTIs are mild, certain symptoms require prompt medical attention.

Difficulty breathing or significant shortness of breath is an important warning sign.

Chest pain, bluish discoloration of the lips, severe weakness, confusion, fainting, or inability to maintain adequate fluid intake should not be attributed automatically to a simple cold.

Severe difficulty swallowing, drooling, muffled voice, marked swelling of the throat or neck, or inability to open the mouth normally may indicate a serious upper-airway or deep-neck complication.

Persistent or rapidly worsening fever, severe unilateral facial swelling, severe headache with neurological symptoms, or significant deterioration after initial improvement also warrants medical evaluation.

In infants and young children, warning signs include poor feeding, reduced urine output, unusual sleepiness, persistent irritability, rapid breathing, chest retractions, grunting, or bluish skin.

URTI in Older Adults and High-Risk Patients

Older adults and individuals with chronic diseases may not always present with classic symptoms.

A patient with significant heart or lung disease may deteriorate more quickly after a respiratory infection. Similarly, immunocompromised patients may develop severe disease with relatively subtle early symptoms.

Patients with chronic respiratory diseases should follow their individualized action plans when respiratory infections trigger worsening symptoms.

People at increased risk of complications should seek medical advice earlier when symptoms are significant, persistent, or worsening.

NICE recognizes systemic illness and higher-risk medical conditions as important factors when deciding whether antibiotic treatment or further assessment may be required.

Prevention of Upper Respiratory Tract Infections

Prevention begins with reducing exposure and interrupting transmission.

Regular hand hygiene is one of the simplest preventive measures. Hands should be washed with soap and water when visibly dirty and after coughing, sneezing, using tissues, or caring for someone who is ill.

Respiratory etiquette is also important. Covering coughs and sneezes with a tissue or the elbow reduces contamination of hands and surrounding surfaces.

Avoiding close contact with people who are acutely ill can reduce transmission.

Good ventilation is particularly useful in indoor environments because respiratory viruses can spread through inhaled infectious particles.

Vaccination provides protection against several important respiratory pathogens. Influenza vaccination and COVID-19 vaccination can reduce the risk of severe disease, while other vaccines may be recommended according to age, medical conditions, pregnancy, occupation, and national immunization schedules.

Smoking and Respiratory Infections

Smoking is an important modifiable risk factor for respiratory disease.

Tobacco smoke damages airway epithelial cells and impairs mucociliary clearance. It can also alter immune responses and increase susceptibility to respiratory infections.

Avoiding smoking and secondhand smoke can therefore contribute to better respiratory health.

Patients who repeatedly develop respiratory infections should be encouraged to consider smoking cessation if they smoke.

Hygiene in Schools and Workplaces

Because URTIs spread readily through close contact, schools and workplaces can play an important role in prevention.

People with acute respiratory illness should minimize close contact with others while they are highly symptomatic and follow current public-health recommendations regarding return to normal activities.

Frequently touched surfaces should be cleaned appropriately when illness is circulating.

Improved ventilation and avoiding overcrowded poorly ventilated spaces can help reduce transmission.
