Introduction

Conjunctivitis is one of the most common disorders affecting the eye and is characterized by inflammation of the conjunctiva, the thin, transparent mucous membrane that covers the anterior surface of the eye and lines the inner surface of the eyelids. Because the conjunctiva is highly exposed to the external environment, it can easily become irritated or infected by microorganisms, allergens, chemicals, foreign substances, and other environmental factors. Conjunctivitis may affect one or both eyes and can occur at any age, from newborn infants to older adults.

The condition is commonly recognized by redness of the eye, excessive tearing, irritation, itching, burning, discharge, and a sensation of having something inside the eye. Although conjunctivitis is frequently mild and self-limiting, its clinical appearance can vary considerably depending on the underlying cause. Viral conjunctivitis may produce watery discharge and significant redness, whereas bacterial conjunctivitis is more commonly associated with thick mucopurulent discharge. Allergic conjunctivitis is typically dominated by intense itching and watery eyes.

Conjunctivitis is important not only because of its high frequency but also because some forms are highly contagious. Viral and bacterial conjunctivitis can spread rapidly through direct contact, contaminated hands, towels, cosmetics, eye drops, and other objects that come into contact with the eyes. Outbreaks may occur in schools, households, workplaces, hospitals, and other crowded environments.

Most cases do not cause permanent visual impairment. However, certain severe infections, particularly those involving the cornea, can threaten vision if diagnosis and treatment are delayed. Therefore, distinguishing uncomplicated conjunctivitis from potentially serious ocular disease is an important clinical skill.

Anatomy of the Conjunctiva

The conjunctiva is a thin, transparent mucous membrane that plays an important protective role in maintaining the health of the ocular surface. It is divided anatomically into three major portions: the palpebral conjunctiva, the bulbar conjunctiva, and the fornicial conjunctiva.

The palpebral conjunctiva lines the inner surface of the upper and lower eyelids. It remains closely attached to the eyelids and helps provide a smooth surface over which the eye can move during blinking.

The bulbar conjunctiva covers the anterior portion of the sclera but does not cover the cornea. Because it is relatively transparent, the underlying white sclera can normally be seen through it.

The fornicial conjunctiva forms the loose folds where the palpebral and bulbar conjunctiva meet. These folds allow the eyelids and eyeball to move freely without excessive tension.

The conjunctiva contains numerous blood vessels, lymphatic vessels, immune cells, goblet cells, and epithelial cells. Its blood supply and immune defenses make it capable of responding rapidly to infection and inflammation.

Goblet cells within the conjunctiva produce mucins that contribute to the tear film. These mucins help maintain a smooth ocular surface and assist in protecting the eye from microorganisms and environmental particles.

Normal Protective Mechanisms of the Eye

The eye possesses several mechanisms that protect it against infection and injury. The eyelids provide mechanical protection, while blinking continuously spreads the tear film across the ocular surface and helps remove foreign particles.

Tears contain several antimicrobial substances, including lysozyme, lactoferrin, immunoglobulins, and other protective proteins. These substances inhibit microbial growth and contribute to the defense of the ocular surface.

The conjunctival epithelium also acts as a physical barrier. When microorganisms or allergens come into contact with the eye, local immune mechanisms can be activated, producing inflammation and recruitment of immune cells.

Despite these protective mechanisms, the conjunctiva remains vulnerable because it is directly exposed to the environment. Viral particles, bacteria, allergens, smoke, dust, chemicals, and other irritants can reach the conjunctival surface relatively easily.

When these protective mechanisms are overwhelmed or the inflammatory response becomes excessive, conjunctivitis can develop.

Definition of Conjunctivitis

Conjunctivitis can be defined as inflammation of the conjunctiva caused by infectious, allergic, chemical, mechanical, or other inflammatory processes.

The condition may be classified according to its cause, duration, clinical presentation, or severity. From a practical clinical perspective, the major categories are:

  • Viral conjunctivitis
  • Bacterial conjunctivitis
  • Allergic conjunctivitis
  • Chemical or toxic conjunctivitis
  • Irritant conjunctivitis
  • Neonatal conjunctivitis
  • Conjunctivitis associated with systemic inflammatory disease

Among these, viral, bacterial, and allergic conjunctivitis account for a large proportion of cases encountered in clinical practice.

Causes of Conjunctivitis

The causes of conjunctivitis are diverse. Infectious causes include viruses and bacteria, while noninfectious causes include allergies, chemical exposure, trauma, foreign bodies, and environmental irritation.

Viral Causes

Viruses are an important cause of acute conjunctivitis. Adenoviruses are particularly well known for causing highly contagious outbreaks.

Other viruses that can cause conjunctivitis include:

  • Herpes simplex virus
  • Varicella-zoster virus
  • Enteroviruses
  • Other respiratory viruses

Viral conjunctivitis often occurs in association with an upper respiratory tract infection. Patients may develop sore throat, nasal congestion, cough, fever, or other respiratory symptoms around the same time as the eye symptoms.

Adenoviral conjunctivitis can spread rapidly between individuals and may affect both eyes sequentially.

Bacterial Causes

Bacterial conjunctivitis occurs when pathogenic bacteria infect the conjunctival surface. Common organisms include:

  • Staphylococcus aureus
  • Streptococcus pneumoniae
  • Haemophilus influenzae
  • Moraxella species

Certain bacteria can produce particularly severe disease. Neisseria gonorrhoeae, for example, may cause rapidly progressive hyperpurulent conjunctivitis and requires urgent treatment because the infection can invade the cornea and potentially result in perforation.

Allergic Causes

Allergic conjunctivitis occurs when the conjunctiva reacts to allergens. Common triggers include:

  • Pollen
  • House dust mites
  • Animal dander
  • Mold
  • Cosmetics
  • Certain medications
  • Environmental allergens

Allergic conjunctivitis may be seasonal or perennial depending on the allergen involved.

Unlike infectious conjunctivitis, allergic conjunctivitis is generally not contagious.

Risk Factors

Several factors increase the likelihood of developing conjunctivitis. Exposure to infected individuals is particularly important for viral and bacterial disease.

This includes viral and bacterial forms. Infectious conjunctivitis is frequently contagious and may spread from one eye to the other.

Noninfectious Conjunctivitis

Noninfectious disease includes allergic, chemical, toxic, and mechanical conjunctivitis. These forms are not generally transmitted from person to person.

Conjunctivitis can also be classified according to duration:

Acute conjunctivitis usually develops suddenly and lasts for a relatively short period.

Chronic conjunctivitis persists for several weeks or recurs repeatedly. Chronic cases may require investigation for underlying allergy, eyelid disease, medication exposure, chronic infection, or systemic inflammatory conditions.

Pathophysiology of Conjunctivitis

The pathophysiology of conjunctivitis depends on the underlying trigger, but the final process involves inflammation of the conjunctival tissues.

When an infectious organism enters the conjunctiva, it may attach to and penetrate susceptible epithelial cells. Recognition of microbial components by the immune system activates inflammatory pathways.

Inflammatory mediators such as cytokines and chemokines are released. These substances increase local blood flow and vascular permeability and attract immune cells to the affected tissue.

The superficial blood vessels of the conjunctiva become dilated, producing the characteristic red appearance of the eye.

Increased vascular permeability contributes to tissue swelling and may produce conjunctival edema, known as chemosis.

Goblet-cell activity and tear production may increase, resulting in watery or mucoid discharge. In bacterial infections, accumulation of inflammatory cells, mucus, epithelial debris, and bacteria can produce a thicker purulent discharge.

In allergic conjunctivitis, exposure to an allergen activates mast cells and causes release of mediators such as histamine. Histamine produces itching, vascular dilation, increased vascular permeability, and stimulation of tear production.

Clinical Features

The clinical presentation of conjunctivitis varies according to its cause, but several symptoms are common.

Eye Redness

Redness is one of the most recognizable findings. It results primarily from dilation of conjunctival blood vessels.

The redness may involve the entire conjunctiva or may be more pronounced in particular areas.

Eye Discharge

Discharge is an important diagnostic clue.

Watery discharge is more typical of viral or allergic conjunctivitis, while bacterial conjunctivitis often produces mucopurulent discharge.

Patients may notice crusting around the eyelids, especially after sleeping.

Itching

Intense itching is particularly characteristic of allergic conjunctivitis. A patient who primarily complains of severe itching should raise suspicion for an allergic cause.

Vigorous rubbing of the eyes can worsen inflammation and may cause additional mechanical irritation.

Burning and Irritation

Patients may describe a burning or gritty sensation. They may feel as though sand or a small foreign object is present in the eye.

Excessive Tearing

Inflammation stimulates reflex tearing. Tearing is particularly prominent in viral, allergic, and irritant conjunctivitis.

Eyelid Swelling

Inflammation can cause mild to moderate swelling of the eyelids. In severe infections, swelling may become substantial and should be evaluated carefully to exclude deeper orbital disease.

Viral Conjunctivitis

Viral conjunctivitis commonly begins in one eye and may spread to the other eye within several days.

Patients often experience:

  • Redness
  • Watery discharge
  • Burning
  • Foreign-body sensation
  • Photophobia
  • Mild eyelid swelling

Associated respiratory symptoms may occur, particularly with adenoviral infections.

A notable feature of viral conjunctivitis is its contagious nature. Viral particles can remain on contaminated hands and surfaces and can be transmitted through eye contact.

Some adenoviral infections can involve the cornea, producing keratoconjunctivitis. Corneal involvement may cause more pronounced photophobia and visual disturbance.

Bacterial Conjunctivitis

Bacterial conjunctivitis commonly produces conjunctival redness accompanied by mucopurulent or purulent discharge.

Patients may report that their eyelids become stuck together after sleeping because of accumulated discharge.

Typical features include:

  • Red eye
  • Thick discharge
  • Eyelid crusting
  • Burning
  • Mild foreign-body sensation
  • Irritation

Visual acuity is usually preserved in uncomplicated conjunctivitis. Significant reduction in vision should prompt consideration of corneal or intraocular involvement.

Bacterial conjunctivitis may initially affect one eye but can subsequently involve both eyes.

Allergic Conjunctivitis

Allergic conjunctivitis is characterized primarily by itching.

Patients commonly have:

  • Intense itching
  • Watery eyes
  • Redness
  • Eyelid swelling
  • Burning
  • Stringy mucus
  • Nasal symptoms

Allergic rhinitis may occur simultaneously, producing sneezing, nasal congestion, and a runny nose.

Patients frequently rub their eyes because of severe itching. Repeated rubbing can worsen the inflammatory response and may contribute to mechanical injury of the ocular surface.

Chemical and Irritant Conjunctivitis

Exposure to chemicals can produce conjunctival inflammation. Common irritants include smoke, dust, chlorine, cleaning products, cosmetics, and industrial chemicals.

The severity depends on the substance, concentration, duration of exposure, and whether the cornea or deeper ocular tissues are affected.

Chemical exposure should be treated seriously because some substances can cause significant ocular surface damage rather than simple conjunctivitis.

Immediate irrigation is an important first-aid measure following significant chemical exposure.

Neonatal Conjunctivitis

Conjunctivitis occurring in newborn infants is called ophthalmia neonatorum or neonatal conjunctivitis.

It may be caused by bacteria acquired during delivery, including Neisseria gonorrhoeae and Chlamydia trachomatis. Other bacterial and viral causes may also occur.

Neonatal conjunctivitis requires careful evaluation because certain pathogens can cause severe ocular disease.

Gonococcal conjunctivitis is particularly dangerous because it can rapidly invade the cornea and cause corneal ulceration or perforation.

The timing of symptom onset after birth can provide useful clues about the possible cause, although laboratory evaluation may be required.

Diagnosis of Conjunctivitis

Diagnosis is usually based on clinical history and ocular examination.

The clinician should determine:

  • Onset and duration of symptoms
  • Whether one or both eyes are involved
  • Nature of discharge
  • Presence of itching
  • Presence of pain
  • Degree of photophobia
  • Changes in vision
  • Recent respiratory illness
  • Contact with infected individuals
  • Contact-lens use
  • Exposure to allergens
  • Chemical exposure
  • History of trauma
  • Previous episodes

A careful examination is essential because several serious ocular conditions can resemble conjunctivitis.

Eye Examination

Visual acuity should be assessed, particularly when symptoms are significant.

The conjunctiva is examined for:

  • Degree of redness
  • Distribution of vascular injection
  • Discharge
  • Chemosis
  • Follicles
  • Papillae
  • Conjunctival membranes or pseudomembranes

The eyelids should also be examined for swelling, crusting, lesions, and signs of inflammation.

The cornea must be evaluated when pain, photophobia, contact-lens use, or visual impairment is present.

Fluorescein staining may be used to identify corneal epithelial defects, abrasions, or ulcers.

Laboratory Investigations

Most uncomplicated cases do not require extensive laboratory testing.

However, microbiological testing may be considered when:

  • The infection is severe
  • The condition is recurrent
  • The patient is immunocompromised
  • There is significant corneal involvement
  • Initial treatment fails
  • A sexually transmitted infection is suspected
  • Gonococcal or chlamydial infection is suspected
  • There is an unusual clinical presentation

Conjunctival swabs can be used for bacterial culture and sensitivity testing when clinically indicated.

Molecular testing may be useful for certain viral or sexually transmitted infections.

Differential Diagnosis

Not every red eye is conjunctivitis. This distinction is extremely important because several potentially vision-threatening conditions can present with ocular redness.

Important differential diagnoses include:

  • Corneal abrasion
  • Keratitis
  • Corneal ulcer
  • Uveitis
  • Acute angle-closure glaucoma
  • Scleritis
  • Episcleritis
  • Foreign body
  • Chemical injury
  • Orbital cellulitis
  • Preseptal cellulitis
  • Severe dry-eye disease

The presence of significant pain, marked photophobia, reduced visual acuity, corneal opacity, fixed or irregular pupil, severe headache, nausea, or other concerning findings should prompt urgent ophthalmic assessment.

Treatment Principles

Treatment depends on the underlying cause. Antibiotics should not automatically be used for every red eye because many cases are viral or allergic.

The major principles of treatment include:

  • Identifying the underlying cause
  • Relieving symptoms
  • Preventing transmission when infection is present
  • Treating bacterial infection when appropriate
  • Controlling allergic inflammation
  • Protecting the cornea
  • Avoiding unnecessary medications
  • Monitoring for complications

Patients should avoid rubbing their eyes and should wash their hands frequently.

Artificial tears may provide symptomatic relief in several forms of conjunctivitis.

Management of Viral Conjunctivitis

Most uncomplicated viral conjunctivitis cases are self-limiting and improve with supportive treatment.

Supportive measures include:

  • Artificial tears
  • Cool compresses
  • Adequate hand hygiene
  • Avoidance of eye rubbing
  • Temporary avoidance of contact lenses
  • Avoidance of sharing towels and cosmetics

Antibiotics do not treat viral infections.

Some specific viral infections, such as herpes simplex or varicella-zoster involving the eye, may require antiviral therapy and specialist management.

Topical corticosteroids should not be used casually because they may worsen certain infections, particularly herpes simplex keratitis, and may cause other ocular complications if used inappropriately.

Management of Bacterial Conjunctivitis

Many uncomplicated bacterial infections may resolve spontaneously, but topical antibiotics can shorten the duration of symptoms in selected cases and may reduce transmission.

The choice of antibiotic depends on the clinical situation and local resistance patterns.

Common topical antibiotic classes include:

  • Aminoglycosides
  • Fluoroquinolones
  • Macrolides
  • Other ophthalmic antibacterial agents

Contact-lens wearers require special consideration because of the increased risk of bacterial keratitis, particularly infections caused by organisms such as Pseudomonas aeruginosa.

A contact-lens wearer with significant pain, photophobia, or visual impairment should receive prompt ophthalmic assessment rather than being treated as uncomplicated conjunctivitis.

Management of Allergic Conjunctivitis

Treatment focuses on avoiding allergens and reducing ocular inflammation.

Helpful measures include:

  • Cold compresses
  • Artificial tears
  • Allergen avoidance
  • Antihistamine eye drops
  • Mast-cell stabilizing agents
  • Combination antihistamine/mast-cell stabilizing preparations when appropriate

For persistent or severe allergic disease, an ophthalmologist may consider additional anti-inflammatory treatment.

Patients should avoid excessive eye rubbing because it can worsen symptoms and cause mechanical damage.

Infection Control and Prevention

Because viral and bacterial conjunctivitis can be contagious, infection-control measures are extremely important.

Contact Lenses and Conjunctivitis

Contact lenses can increase the risk of ocular infection when they are not used or cleaned properly.

Patients with conjunctivitis should generally stop wearing contact lenses until the eye has recovered and a healthcare professional has advised that lens use can safely resume.

Important preventive measures include:

  • Washing hands before handling lenses
  • Using recommended cleaning solutions
  • Replacing lens cases regularly
  • Avoiding sleeping in lenses unless specifically prescribed
  • Avoiding swimming while wearing lenses when appropriate

Complications of Conjunctivitis

Most cases resolve without permanent complications, but complications can occur depending on the underlying cause.

Conjunctivitis is common among children because of frequent hand-to-eye contact and close interaction with other children.

Children may have difficulty describing symptoms and may primarily present with:

  • Red eyes
  • Discharge
  • Eyelid crusting
  • Irritability
  • Frequent eye rubbing

Parents and caregivers should emphasize hand hygiene and avoid sharing towels, pillows, or eye-care products.

A child with significant pain, light sensitivity, visual changes, severe swelling, or an abnormal-looking cornea requires medical evaluation.

Conjunctivitis in Immunocompromised Patients

Patients with impaired immune function may develop more severe or persistent infections.

In such patients, clinicians should maintain a lower threshold for microbiological investigation and ophthalmic referral.

Unusual organisms, recurrent infections, prolonged disease, and poor response to standard therapy should prompt consideration of an underlying immune or systemic problem.

Prognosis

The prognosis of uncomplicated conjunctivitis is generally excellent.

Viral conjunctivitis commonly resolves gradually without permanent visual impairment. Bacterial conjunctivitis also usually responds well to appropriate management, while allergic conjunctivitis may recur whenever the patient encounters the responsible allergen.

The prognosis depends strongly on whether the cornea is involved. Simple conjunctival inflammation usually does not damage vision, whereas severe keratitis or corneal ulceration can lead to scarring and visual impairment.

Early recognition of serious warning signs therefore remains an important part of managing patients with red eyes.

Long-Term Prevention

Preventing conjunctivitis requires attention to hygiene, environmental exposure, contact-lens practices, and allergy control.

Regular handwashing is one of the simplest and most effective preventive measures against infectious conjunctivitis.

Individuals should avoid touching their eyes unnecessarily and should not share personal items that come into contact with the face or eyes.

People with recurrent allergic conjunctivitis can benefit from identifying and reducing exposure to specific allergens.

Proper contact-lens hygiene is essential for preventing both conjunctivitis and more serious corneal infections.

Maintaining good eyelid hygiene and avoiding contaminated cosmetics can further reduce ocular irritation and infection.

Clinical Importance

Although conjunctivitis is often considered a minor eye disorder, it represents a broad group of conditions with different causes, transmission patterns, treatments, and potential complications.

The clinician must therefore avoid diagnosing every red eye as simple conjunctivitis. The history, nature of discharge, severity of pain, presence of photophobia, visual acuity, corneal examination, contact-lens history, and systemic symptoms all contribute to accurate diagnosis.

A watery, itchy red eye associated with allergy is clinically different from a painful red eye with reduced vision. Similarly, a mild bacterial infection differs substantially from rapidly progressive hyperpurulent conjunctivitis caused by Neisseria gonorrhoeae.

Understanding these distinctions allows appropriate treatment, reduces unnecessary antibiotic use, prevents transmission, and helps identify patients who require urgent ophthalmologic care.

Patient Education

Patient education is an essential component of conjunctivitis management.

Patients should understand that the appearance of a red eye does not automatically mean that antibiotic treatment is necessary. Treatment must be directed toward the underlying cause.

Patients with suspected infectious conjunctivitis should be advised about hand hygiene and avoiding close contact involving the eyes. They should also avoid sharing towels, cosmetics, eye drops, and other personal items.

Patients should be instructed to return for medical assessment if symptoms worsen, vision becomes blurred or reduced, pain increases, or significant photophobia develops.

Appropriate education not only improves recovery but also helps prevent transmission and reduces the risk of serious complications.

Ongoing Clinical Assessment

Persistent or recurrent conjunctivitis should not simply be treated repeatedly without determining the underlying cause.

Chronic symptoms may indicate:

  • Allergic disease
  • Chronic blepharitis
  • Dry-eye disease
  • Medication-related irritation
  • Chronic infection
  • Eyelid abnormalities
  • Contact-lens complications
  • Systemic inflammatory disease

When symptoms do not follow the expected course, the diagnosis should be reconsidered.

A careful ocular examination and appropriate referral can prevent prolonged discomfort and identify diseases that may otherwise remain undiagnosed.

Histological Changes in Conjunctivitis

The inflammatory response in conjunctivitis produces characteristic changes within the conjunctival epithelium and underlying tissues. The exact histological pattern depends on the cause and duration of inflammation.

In acute conjunctivitis, vascular dilation and increased blood flow produce prominent conjunctival hyperemia. Increased vascular permeability allows fluid and inflammatory proteins to move into surrounding tissues, which can produce conjunctival edema or chemosis.

Inflammatory cells may accumulate beneath the conjunctival epithelium. Neutrophils are commonly prominent in acute bacterial inflammation, while lymphocytes may be more characteristic of many viral infections. Allergic disease may demonstrate eosinophilic and mast-cell activity.

The conjunctival epithelium may also undergo reactive changes. Increased mucus production can contribute to discharge, while prolonged inflammation may alter goblet-cell function and the stability of the tear film.

These microscopic changes explain many of the visible clinical findings, including redness, swelling, discharge, irritation, and excessive tearing.

Follicular and Papillary Reactions

Follicles and papillae are important clinical findings during examination of the conjunctiva.

A follicle is a small, rounded elevation produced by lymphoid tissue within the conjunctiva. Follicular reactions are commonly associated with viral conjunctivitis and certain chronic infections.

A papilla is a raised conjunctival structure with a central blood vessel and surrounding inflammatory tissue. Papillary reactions are commonly associated with allergic conjunctivitis and mechanical irritation.

Large papillae may occur in conditions such as vernal keratoconjunctivitis and giant papillary conjunctivitis associated with contact-lens use.
