Analgesics and Sedatives


1. _Morphine_

    - Dose: 0.05-0.2 mg/kg IV bolus, followed by 0.01-0.05 mg/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Pain management, sedation

    - Indication: Acute pain, agitation

    - Side effects: Respiratory depression, hypotension, constipation

    - Antidote: Naloxone (0.4-2 mg IV)

2. _Fentanyl_

    - Dose: 0.5-2 mcg/kg IV bolus, followed by 0.5-2 mcg/kg/h IV infusion

    - Use: Pain management, sedation

    - Side effects: Respiratory depression, hypotension, bradycardia

    - Antidote: Naloxone (0.4-2 mg IV)

3. _Midazolam_

    - Indication: Systolic BP < 90 mmHg

    - Side effects: Hypertension, tachycardia, peripheral ischemia

    - Antidote: None

2. _Epinephrine_

    - Dose: 0.01-1 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Hypotension, cardiac arrest

    - Indication: Systolic BP < 90 mmHg, cardiac arrest

    - Side effects: Hypertension, tachycardia, arrhythmias

    - Antidote: None

3. _Vasopressin_

    - Dose: 0.01-0.04 U/min IV infusion

    - Administration: IV infusion

    - Use: Hypotension, septic shock

    - Indication: Systolic BP < 90 mmHg

    - Side effects: Hypertension, hyponatremia, peripheral ischemia

    - Antidote: None


Inotropes


1. _Dobutamine_

    - Dose: 2-20 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Cardiac failure, cardiogenic shock

    - Indication: Low cardiac output, hypotension

    - Side effects: Tachycardia, hypertension, arrhythmias

    - Antidote: None

2. _Milrinone_

    - Dose: 0.125-0.5 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Cardiac failure, cardiogenic shock

    - Indication: Low cardiac output, hypotension

    - Side effects: Hypotension, tachycardia, arrhythmias

    - Antidote: None


Antiarrhythmics


1. _Lidocaine_

    - Dose: 1-4 mg/kg IV bolus, followed by 1-4 mg/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Ventricular arrhythmias

    - Indication: VT, VF

    - Side effects: CNS toxicity, cardiac depression

    - Antidote: None

2. _Amiodarone_

    - Dose: 5-10 mg/kg IV bolus, followed by 1-2 mg/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Ventricular arrhythmias, atrial fibrillation

    - Indication: VT, VF, AF

    - Side effects: Hypotension, bradycardia, thyroid dysfunction

    - Antidote: None


Antihypertensives


1. _Nitroglycerin_

    - Dose: 0.5-10 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Hypertension, angina

    - Indication: Systolic BP > 180 mmHg

    - Side effects: Hypotension, headache, methemoglobinemia

    - Antidote: None

2. _Labetalol_

    - Dose: 0.1-1 mg/kg IV bolus, followed by 0.01-0.1 mg/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Hypertension

    - Indication: Systolic BP > 180 mmHg

    - Side effects: Hypotension, bradycardia, bronchospasm

    - Antidote: None

3. _Nicardipine_

    - Dose: 0.5-5 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Hypertension

    - Indication: Systolic BP > 180 mmHg

    - Side effects: Hypotension, tachycardia, peripheral edema

    - Antidote: None

4. _Clevidipine_

    - Dose: 0.5-16 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Hypertension

    - Indication: Systolic BP > 180 mmHg

    - Side effects: Hypotension, tachycardia, headache

    - Antidote: None


Anticoagulants


1. _Heparin_

    - Dose: 50-100 U/kg IV bolus, followed by 10-20 U/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Thrombosis, embolism

    - Indication: DVT, PE, AF

    - Side effects: Bleeding, thrombocytopenia

    - Antidote: Protamine (1-2 mg IV)

2. _Warfarin_

    - Dose: 0.1-0.3 mg/kg PO daily

    - Administration: Oral

    - Use: Thrombosis, embolism

    - Indication: DVT, PE, AF

    - Side effects: Bleeding, skin necrosis

    - Antidote: Vitamin K (1-2 mg IV)

3. _Fondaparinux_

    - Dose: 2.5-5 mg SC daily

    - Administration: Subcutaneous injection

    - Use: Thrombosis, embolism

    - Indication: DVT, PE

    - Side effects: Bleeding, thrombocytopenia

    - Antidote: None


Antiplatelet Agents


1. _Aspirin_

    - Dose: 50-100 mg PO daily

    - Administration: Oral

    - Use: Thrombosis, embolism

    - Indication: MI, stroke, TIA

    - Side effects: Bleeding, GI upset

    - Antidote: None

2. _Clopidogrel_

    - Dose: 75 mg PO daily

    - Administration: Oral

    - Use: Thrombosis, embolism

    - Indication: MI, stroke, TIA

    - Side effects: Bleeding, thrombotic thrombocytopenic purpura

    - Antidote: None


Vasodilators


1. _Nitroprusside_

    - Dose: 0.5-10 mcg/kg/min IV infusion

    - Administration: IV infusion

    - Use: Hypertension, heart failure

    - Indication: Systolic BP > 180 mmHg

    - Side effects: Hypotension, cyanide toxicity

    - Antidote: None

2. _Hydralazine_

    - Dose: 0.1-0.5 mg/kg IV bolus, followed by 0.01-0.05 mg/kg/h IV infusion

    - Administration: IV injection or infusion

    - Use: Hypertension

    - Indication: Systolic BP > 180 mmHg
